How to Register Your Child in the Levittown School District

The following is a list of items that you will need to successfully register your child in the Levittown Public School District.

What does my child need in order to attend a Levittown School?
Please be aware that the Levittown Public School District maintains the right to verify all of the information presented upon application for registration. This may include but is not limited to home visits to verify residency.

I. - PROOF OF IMMUNIZATION

Required:

  • Certificate of Immunization
  • Current Physical Examination - signed by a physician
  • Medical History form

II. - BIRTH CERTIFICATE

Required:

  • Original document
    Copies will be made at our office for the school district files

III. - REPORT CARDS

Required:

  • Report card from last school year of previous school

IV. - Proof of Residency

Proof of residency must be established at the time of registration. The District will rely on your representations with respect to residency. Please be advised you will be legally responsible for any inaccuracy or misrepresentation of the facts under 210.45 of the Penal Law of the State of New York as well as the payment of tuition and attorneys fees.

Please provide original documents. Copies will be made at our office for our files.

IF YOU OWN YOUR HOME THE FOLLOWING IS REQUIRED:

  1. A. Affidavit of Residency (notarized by you)

    In addition:

    Two (2) Bills - Any of the following is acceptable

    • Electric Bill (Keyspan, LIPA)
    • Telephone Bill
    • Water Bill
    • Cable Bill
    • Bank Statement or copy of blank check with address printed on it


  2. Owner's Affidavit (notarized by owner of home)

    In addition:

    One (1) of the following

    • Owner's Deed
    • Closing Statement
    • Mortgage Statement
    • Tax Statement

IF YOU RENT YOUR HOME THE FOLLOWING IS REQUIRED:

  1. Affidavit of Residency (notarized by you)

    In addition:

    Two (2) Bills - Any of the following is acceptable

    • Electric Bill (Keyspan, LIPA)
    • Telephone Bill
    • Water Bill
    • Cable Bill
    • Bank Statement or copy of blank check with address printed on it


  2. Owner's Affidavit (notarized by owner of home)

    In addition:

    One (1) of the following

    • Owner's Deed
    • Closing Statement
    • Mortgage Statement
    • Tax Statement


  3. Renter/Non-Owner Affidavit (notarized by you)

    In addition:

    • Rental Agreement
    • Lease
    • 3 pieces of official mail recently sent via the United States Postal Service
      (in the postmarked envelope)

Proof of Guardianship or Custodianship:
Can be shown through divorce or other court papers.
(Additional documentation will be given at time of registration)

All supplementary proofs must bear your name and must indicate your Levittown Public School District address.
